A Planetree Designated Hospital
The Importance of Planetree Designation
Planetree designation represents the highest level of achievement in patient/person-centered care based on evidence and standards.
At a time when healthcare marketing campaigns increasingly assert organizations are pledging to “put patients first,” the Planetree Designation Program is the only program to formally recognize excellence in patient- and person-centered care across the continuum of care.
The Designation Program provides a structured, operational framework for evaluating the organizational systems and processes necessary to sustain organizational culture change. Through a set of experience-based and evidence-based criteria, the program converts the aspirational aim of becoming more “patient-centered” into something that is defined, attainable and measurable.
Being a Planetree Designated Hospital
Longmont United Hospital has demystified the healthcare experience for the patient and raised the expectations of care. Each patient can expect each health professional to consider his or her comfort and dignity while providing top-quality care. Each patient is encouraged to be actively involved in learning about his or her health and care through patient education and review of medical charts. The Hospital also expects health professionals from different disciplines to provide continuity of care through communication to each other and the patient. Longmont United Hospital implemented weekly multidisciplinary rounds to facilitate communication.

Health Center of Integrated Therapies, Longmont United Hospital's integrated therapies service, provides a range of therapies including stress reduction or pain management tools such as acupuncture, massage, and meditation. Providing excellent patient-centered care begins with excellent employees and physicians. Longmont United Hospital offers opportunities, such as the Embrace Wellness program, to help each employee have a well-rounded life while being mentally, socially, and physically healthy. The Hospital believes each employee needs to take care of him or her self in order to provide excellent and compassionate care.
Improving the health of the communities served is part of the Longmont United Hospital’s Mission and Environmental Statement. A composting and recycling program was implemented throughout the Hospital along with a medication take-back program that prevented medications going into the wastewater stream. Longmont United Hospital has received several community awards for leadership in business practices to reduce waste and pollution.
